Who are entitled to Get Medical Certificate from Military Hospital
In many occasions, it has been found that there is an extreme need for obtaining a medical certificate and disability for the dependents of veterans and serving soldiers. It has been noticed that the exact process and documentation to be submitted is a big issue as there are no such specific uniform Govt SOP in this regard. However, the standard process and documentation as followed by most of the Military hospitals is reproduced here.
How to apply for Medical Certificate from Military Hospital/ ECHS
Military Hospitals are responsible to issue medical certificate only for the Serving soldiers and their dependents. For veterans and their dependents, they should approach to the ECHS Polyclinic. Documents required to obtain a medical certificate is given here
Documents Required to get Medical Certificate from Military Hospitals
- A personal application addressed to the Comd, Concerned MH, duly stated the purpose for the medical certificate and must be recommended by OC Unit (for serving soldiers). For ECHS Beneficiaries, submit application directly to the ECHS Parent Polyclinic. Must mention your Mobile Number.
- Copy of Aadhar Card of the Dependent.
- Copy of dependent card/ MEC for dependents of serving soldiers. For ECHS Beneficiaries, copy of ECHS card to be submitted
- Copy of all relevant medical documents / treatment related documents.
